Chronic Disease Management Plans
If you suffer a chronic medical condition, you may be able to get Medicare benefits to cover allied health services.
A chronic medical condition is one that has been, or is likely to be present for 6 months or more, or is terminal in nature. Chronic diseases include but are not limited to cancer, asthma, diabetes, cardiovascular disease, stroke and musculoskeletal conditions.
If you GP believes your condition requires the services of other health professionals
they may set up a Team Care Arrangement, in which services needed may be
covered by Medicare. (Example Podiatry, counselling, audiologists, dieticians,
diabetes educators, mental health workers, occupational therapists, osteopaths,
physiotherapists, speech pathologists, and Aboriginal health workers).
Under this Team Care Arrangement, you will be eligible for up to 5 health services each calendar year.
These plans are designed for patients who require a structured approach to their care, and to enable their doctors to plan and coordinate their care requiring ongoing support from a multidisciplinary team.
